RÈGLEMENTS DU CONCOURS

1. ORGANISATEUR

• Le concours « Gagnez votre achat / Gagnez 500$ en carte-cadeau » est organisé par Mode Choc, dont le siège social est situé à Alma, Québec.

2. DURÉE DU CONCOURS

• Le concours débute le 21 novembre 2025 à 00 h 01 (HE) et se termine le 27 novembre 2025 à 23 h 59 (HE).
• La date limite pour soumettre le formulaire de participation est le 28 novembre 2025 à 23 h 59 (HE).

3. ADMISSIBILITÉ

• Le concours s’adresse aux résidents du Québec et du Nouveau-Brunswick âgés de 18 ans et plus.
• Sont exclus : les employés de Mode Choc, ainsi que les personnes résidant sous le même toit qu’eux.
• Pour le concours « Gagnez votre achat » le participant doit avoir enregistré la vente à son propre nom. Les ventes rapides, les comptes à recevoir ainsi que toute transaction effectuée par un organisme ne sont pas admissibles.

4. COMMENT PARTICIPER

Deux options de participation sont offertes :

Option 1 – Gagnez votre achat et/ou 500$ en carte-cadeau

• Effectuez un achat en magasin ou en ligne entre le 21 et le 27 novembre 2025.
• Remplissez le formulaire de participation en ligne avant le 28 novembre 2025 à 23 h 59 (HE), en inscrivant le numéro de facture ou de commande.
• Vous serez inscrit(e) au tirage « Gagnez votre achat » de votre lieu d'achat et automatiquement inscrit(e) au tirage «Gagnez 500 $ en carte-cadeau ».

Option 2 – Gagnez 500$ en carte-cadeau (aucun achat requis)

• Remplissez le formulaire de participation en ligne entre le 21 et le 28 novembre 2025 à 23h59 (HE) et sélectionnez l’option « Aucun achat ».
• Vous serez inscrit(e) au tirage «Gagnez 500 $ en carte-cadeau ».
• Une seule participation par personne est autorisée pour toute la durée du concours.

5. PRIX

5.1 Concours Gagnez votre achat

Douze (12) prix seront attribués :

Un (1) gagnant par magasin (pour chacun des 11 magasins Mode Choc) :

• Un (1) gagnant parmi les achats effectués en ligne au modechoc.ca.
• Chaque gagnant recevra le remboursement complet de la valeur de son achat (incluant les taxes) sans montant maximum.
• L’achat soumis doit être valide, non annulé, ni remboursé au moment du tirage.

5.2 Concours Gagnez 500$ en carte-cadeau

• Un (1) prix sera attribué parmi toutes les participations admissibles, aucun achat requis :
• Un (1) gagnant de 500 $ en carte-cadeau Mode Choc (soit deux cartes-cadeaux de 250 $ chacune).

5.3 Valeur totale approximative des prix

• La valeur totale approximative des prix offerts est composée de 500 $ (deux cartes-cadeaux de 250 $ chacune) plus la valeur variable des achats remboursés dans le cadre du concours « Gagnez votre achat ».

5.4 Conditions relatives aux prix

• Les prix ne sont ni transférables, ni monnayables, ni échangeables contre tout autre bien ou service.

6. TIRAGE

• Les tirages auront lieu le 2 décembre 2025 au siège social de Mode Choc à Alma (Québec), parmi toutes les participations admissibles reçues pendant la période du concours.
• Les gagnants seront contactés par courriel et/ou téléphone dans les 10 jours ouvrables suivant le tirage.
• L’annonce officielle des gagnants sera diffusée sur les comptes Instagram et Facebook de Mode Choc, en story, une fois que tous les gagnants auront été contactés.

7. QUESTION D’HABILETÉ MATHÉMATIQUE

• Pour être déclaré gagnant, chaque participant sélectionné devra répondre correctement à une question d’habileté mathématique posée dans le formulaire d’inscription.

8. REMISE DES PRIX

8.1 Concours Gagnez votre achat

• Le remboursement du prix se fera par chèque, émis au montant total de l’achat gagnant.
• Les gagnants seront contactés par les gestionnaires du magasin Mode Choc concerné lorsqu’il sera temps de récupérer leur prix (prévoir un délai de traitement et d’expédition avant la réception du chèque en magasin).
• Si un gagnant se trouve dans l’impossibilité de se présenter en magasin pour récupérer son prix, le chèque pourra lui être expédié directement à son adresse postale.
• Pour le gagnant ayant effectué son achat en ligne, le chèque sera expédié directement à son adresse postale ou au magasin Mode Choc de son choix.

8.2 Concours Gagnez 500$ en carte-cadeau

• Le prix, soit 500 $ en carte-cadeau (deux cartes-cadeaux d’une valeur de 250 $ chacune), sera remis au gagnant dans le magasin Mode Choc de son choix ou expédié par la poste si la remise en magasin est impossible.

8.3 Conditions pour la remise des prix

• Avant de recevoir son prix, chaque gagnant devra fournir une preuve d’identité valide avec photo.

9. PROTECTION DES RENSEIGNEMENTS PERSONNELS

• Les renseignements personnels recueillis (nom, adresse, courriel, téléphone) sont utilisés uniquement pour l’administration du concours, la communication avec les gagnants et la remise des prix, conformément à la législation en vigueur et à la Politique de confidentialité de Mode Choc.
• En participant, le participant consent à l’utilisation de ses informations aux fins prévues.
• Les données seront conservées uniquement pour la durée nécessaire à l’administration du concours et ne seront ni vendues ni partagées.

10. CONDITIONS GÉNÉRALES

• Mode Choc se réserve le droit de vérifier l’admissibilité des participants et de disqualifier toute participation incomplète, frauduleuse ou non conforme au présent règlement.
• Mode Choc, ses employés, mandataires et fournisseurs de services, ne peuvent être tenus responsables de tout problème technique, erreur d’impression, perte de données ou tout autre événement hors de leur contrôle pouvant affecter la participation ou l’administration du concours.
• Mode Choc se réserve le droit, à son entière discrétion, de modifier, de suspendre ou d'annuler le concours si un événement indépendant de sa volonté en empêche le bon déroulement.

11. DISPONIBILITÉ DU RÈGLEMENT

• Le présent règlement complet est disponible sur le site modechoc.ca.
• Une copie imprimée peut être obtenue sur demande, en magasin ou par courriel.

CONTEST RULES

1. ORGANIZER

• The “Win Your Purchase / Win a $500 Gift Card” contest is organized by Mode Choc, whose head office is located in Alma, Quebec.

2. CONTEST PERIOD

• The contest begins on November 21, 2025 at 12:01 a.m. (ET) and ends on November 27, 2025 at 11:59 p.m. (ET).The deadline to submit the entry form is November 28, 2025 at 11:59 p.m. (ET).

3. ELIGIBILITY

• The contest is open to residents of Quebec and New Brunswick who are 18 years of age or older.
• Excluded: employees of Mode Choc and any individuals residing at the same address as them.
• For the “Win Your Purchase” contest, the participant must have registered the sale under their own name. Quick sales, accounts receivable, and any transaction carried out by an organization are not eligible.

4. HOW TO ENTER

Two entry options are available:

Option 1 – Win Your Purchase and/or $500 in Gift Cards

• Make an in-store or online purchase between November 21 and 27, 2025.
• Complete the online entry form before November 28, 2025 at 11:59 p.m. (ET), entering the invoice or order number.
• You will be entered in the “Win Your Purchase” draw for your purchase location and automatically entered in the “Win a $500 Gift Card” draw.

Option 2 – Win a $500 Gift Card (no purchase required)

• Complete the online entry form between November 21 and 28, 2025 at 11:59 p.m. (ET) and select the “No Purchase” option.
• You will be entered in the “Win a $500 Gift Card” draw.
• Only one entry per person is allowed for the entire contest period.

5. PRIZES

5.1 Win Your Purchase Contest

Twelve (12) prizes will be awarded:

• One (1) winner per store (for each of the 11 Mode Choc stores);
• One (1) winner among the purchases made online at modechoc.ca.
• Each winner will receive a full refund of their purchase (including taxes) with no maximum amount.
• The submitted purchase must be valid, not cancelled, and not refunded at the time of the draw.

5.2 Win a $500 Gift Card Contest

• One (1) prize will be awarded among all eligible entries; no purchase required:
• One (1) winner of a $500 Mode Choc gift card (two $250 gift cards).

5.3 Approximate Total Prize Value

• The approximate total value of the prizes consists of $500 (two $250 gift cards) plus the variable value of the refunded purchases awarded in the “Win Your Purchase” contest.

5.4 Prize Conditions

• Prizes are non-transferable, non-refundable, and cannot be exchanged for any other goods or services.

6. DRAW

• The draws will be held on December 2, 2025 at Mode Choc’s head office in Alma, Quebec, from among all eligible entries received during the contest period.
• Winners will be contacted by email and/or phone within 10 business days following the draw.The official announcement of winners will be posted in Mode Choc’s
• Instagram and Facebook Stories once all winners have been contacted.

7. SKILL-TESTING QUESTION

• To be declared a winner, each selected participant must correctly answer a mathematical skill-testing question asked on the entry form.

8. PRIZE AWARDING

8.1 Win Your Purchase Contest

• The refund will be issued by cheque, made out for the full amount of the winning purchase.
• Winners will be contacted by managers of the relevant Mode Choc store when their prize is ready for pickup (allow time for processing and shipping before the cheque is available in store).
• If a winner is unable to visit the store, the cheque may be mailed directly to their home address.
• For winners who made their purchase online, the cheque will be mailed directly to their home address or to the Mode Choc store of their choice.

8.2 Win a $500 Gift Card Contest

• The prize—$500 in gift cards (two $250 cards)—will be provided to the winner at the Mode Choc store of their choice or mailed if in-store pickup is not possible.

8.3 Prize Claim Requirements

• Before receiving their prize, each winner must provide valid government-issued photo ID.

9. PROTECTION OF PERSONAL INFORMATION

• The personal information collected (name, address, email, phone number) is used solely for contest administration, winner communication, and prize distribution, in accordance with applicable laws and Mode Choc’s Privacy Policy.
• By entering, participants consent to the use of their information for these purposes.
• Data will be retained only for the duration required for contest administration and will not be sold or shared.

10. GENERAL CONDITIONS

• Mode Choc reserves the right to verify participant eligibility and to disqualify any incomplete, fraudulent, or non-compliant entry.
• Mode Choc, its employees, agents, and service providers cannot be held responsible for any technical issues, printing errors, data loss, or other events beyond their control that may affect participation or contest administration.
• Mode Choc reserves the right, at its sole discretion, to modify, suspend, or cancel the contest if an event beyond its control prevents its proper execution.

11. AVAILABILITY OF THE RULES

• The full contest rules are available on the modechoc.ca. website.
• A printed copy can be obtained upon request, in-store or by email.
