In these Terms and Conditions:

Conditions of entry:

  1. Statewide Super directors, staff and their immediate families are not eligible to participate in the
    Heroes Awards (whether as nominator, nominee or by voting).
  2. Entries must be received by 5.00pm Friday, 17 November 2017.
  3. Entries that fail to comply with these Terms and Conditions, or that are late, incomprehensible or
    incomplete, will be deemed invalid.
  4. All nominees must be:
    1. Northern Territory residents of good character
    2. have carried out the nominated activities in the Northern Territory
    3. be an individual (groups and couples are not eligible to be nominated together); and
    4. be alive at the time of nomination (awards will not be granted posthumously).

Prizes:

  1. The total prize pool is $6,000.
  2. Two prizes will be awarded as follows:
    1. Heroes Award Metro – $2,000 plus $1,000 to a charity/organisation of their choice
    2. Heroes Award Regional – $2,000 plus $1,000 to a charity/organisation of their choice

The application process:

  1. To enter the Heroes Awards, the nominator is required to complete and submit an entry form via
    www.statewide.com.au/heroes. Self-nominations are both permissible and welcome.
  2. Entries are treated as non-confidential and non-proprietary and may be published on Statewide Super’s website and social media platforms and in its communications.

Application judging:

  1. Each nominee will be assessed on the extent to which he or she has:
    1. provided physical, emotional or psychological support to a community member/s
    2. displayed commitment to the health and wellbeing of a community member/s
    3. demonstrated an act of courage or exceptional judgment
    4. acted without remuneration or personal gain; and
    5. gone over and above what a ‘reasonable person’ is expected to do.
  2. Statewide Super will select the top 10 nominees (each a Finalist).
  3. A judging panel (comprised of 3 individuals nominated by Statewide Super) will then select the Heroes Awards winners (metro and regional) from among the Finalists.
  4. The outcome of the panel’s decision is final and binding. No correspondence will be entered into.

Winners:

  1. Each winner:
    1. will be contacted by phone or email on or before Tuesday, 28 November 2017 and must give their consent to Statewide Super to use photographs and/or sound and video footage of them by signing the ‘Winner Acceptance Form’ provided to the winner by Statewide Super.
    2. If a Winner fails to provide a signed Winner Acceptance Form to Statewide Super prior to 5pm Tuesday, 28 November 2017, the Winner may be disqualified and an alternate Winner selected by the judging panel to take his or her place.

Privacy:

  1. Statewide Super is collecting nominators and nominees’ personal information for the purpose of
    conducting and promoting the Heroes Awards.
  2. Personal information may be:
    1. disclosed to representatives of Statewide Super and to individuals assisting Statewide Super administer the Heroes Awards; and
    2. used by Statewide Super in accordance with these Terms and Conditions and Statewide
      Super’s privacy policy which can be viewed at
      www.statewide.com.au.

General terms:

  1. Statewide Super may, in its absolute discretion, at any time and without prior notice:
    1. vary any of these terms and conditions
    2. terminate the Heroes Awards; and/or
    3. disqualify a nominee, Finalist or winner.
  2. To the maximum extent permitted by law, Statewide Super excludes liability for any claims in
    connection with the exercise of its rights pursuant to paragraph 16, the Heroes Awards and the
    lateness or loss of an entry or Nominee Acceptance Form.
  3. Each nominator and nominee indemnifies Statewide Super and its officers and employees in respect
    of, and must pay on demand, any loss incurred or suffered in connection with the Heroes A
    wards.
